need help for database design system for record management.

Do you have a basic idea of what you want to do, and you're looking for suggestions/critique on it, or do you need to hire someone to design the database for you from the ground up?

i want to create a system for patient database system.
just wondering what softwares to use.

Having just searched the Internet for inspiration on a schema for remodeling the 'person' in a intelligence system I believe that you will find quite a bit of info out there with a Google search on database schemas for patients.

and also, i want the staff to access files even in their remote offices within the hospital

This does not sound to me like something that you will be building yourself. It may seem like a simple idea, but, trust me, if it has anything to do with medical data and patients, you're going to want to use something built by experienced people.
If you're in the US, the database and software will have to be HIPAA-compliant. Other countries probably have similar regulations. Well, except the UK, where the government seems to have a requirement to leave laptops and such in taxis, trains, etc., with vast amounts of what should be private information.
Or am I misreading what you want?
