I have a SQL 2005 Clustered Server with Database Mail configured. Test Email to a gmail account works with no issue.
Created an alert to monitor for failed logins. It shows an increasing count.
Added notification to the alert to email when the alert fires. No erros but also no email get sent.
SQL Server and SQL Server Agent are using the same service account and it is a member of the local admin as well as SQL SysAdmin.
Mail service reports started.
Queue Mail state is InActive with 0 Length.

Note: Only change lately, was applying SP3 to SQL.

After setting up the alerts notification within the SQL Agent, did you restart the SQL Agent? If you did not, you will need to before emails will be sent for alert or for completed/failed jobs.

I could have sworn I restarted the Agent several times. But I must have missed that step.
