Hi ,
I have 2 node Active/Passive Cluster with sql server 2005. I could see today a failover has happend , but out of 3 instances
only 2 got failedover and one is still there on different node. On checking the logs i can see that failover happened
due to server restart. Can anyone please advise what could have happend with the 3 instance which didnt failedover. I have checked logs also
but couldnot find anyting there as well.

Have you verifyed through event veiwer that the physical server did restart?
It may have been that that 1 sql server (instance) rebooted. If all looks ok, then you should look at the cluster group dependancies for the other instances, there may be a misconfiguration.
Should probably schedule some downtime for test failovers and restarts.

This article has a good section on testing failovers in clusters. Also might be worth reading through and double checking you config.
