It appears that if you specify the columns for a flat file connection manager, and then save those settings, you can't ever go back and add or delete columns. The only solution is to reset the columns and start over.
Is that correct or am I missing something?

Sure you can change the layout, else it would sorta defeat the purpose of development.
Double click on the connection manager and have a look at the advanced tab. There is New Column, and Remove Column options

I had tried that without success.
I have since discovered the problem. When you add/insert a column it defaults the Input Column Width to zero. You must immediately change that value. Otherwise you get the error "Input column widths are ignored because they contain zeroes or negative values." and all of your column settings are reset.
