I have had no issues with SP4. Are the backups when done thru Windows 2000 utility the same size as what would be produced by SQL Server? If not you may not be reaching the point on the tape where the bad block is. Also, check the event viewer logs, they sometimes have additional information that is different from what is stated in SQL and is generally more accurate. I keep getting cannot access in when Event Viewer states drive needs cleaning tape. Other than that try doing a small backup to the tape and see if works. If does then something is an issue to SQL with a spot on the tape that W2K utility is missing, I suggest try another tape. If a new tape fails as well then SQL is having issues with either the driver or the drive.
