  How can we setup auto notification for Availabilty Group Failover in SQL 2012? Please suggest.

  Haven't come across auto notification

  Can make use of this DMV to sys.dm_hadr_availability_group_states , we can look at values for primary_recovery_health and primary_recovery_health_desc which will have the node name.
